Prime Care Mission:

Prime Care Medical Center desires to be the medical home for our patients. The Patient-Centered Medical Home takes those first two words to heart. Patient centered refers to an ongoing, active partnership with your personal physician who leads an exception team of caring professionals dedicated to providing proactive, preventive and chronic care management through all stages of your life. We will focus on the whole patient, physical, emotional, psychological and spiritual. The Prime Care physicians and Family Nurse Practitioners will help navigate you through this complicated healthcare system. We can do this by supplying your family with high quality primary healthcare. When other healthcare professionals are needed we can coordinate care with subspecialists, pharmacists, physical therapists, home health and any other services you may need. The new information technology allows your team to stay up to date on your personal health information and the most recent evidence based treatments available.

We know that you may need medical care or information anytime. We strive to be available when you need us. We have expanded our office hours:

Monday –Thursday: 7:30AM until 8:00PM
Friday: 7:30AM until 5:00PM
Saturday: 8:00AM until 12:00 Noon

Prime Care provides an after-hours call service in which you can talk directly to a registered nurse. You can call the regular office number 731-645-7932 and follow the instructions to talk with the nurse. A Prime Care physician is on call at all times and can be reached by the call service as needed.

We now have a way for you to contact the practice by email. First you must register at one of or Prime Care locations to receive your pin number. Then you may utilize this website (https://portal.pcmedctr.com/portal/) to access your lab information, and x-ray results, to request appointments, referrals and even non narcotic prescription refills. .
